Headaches

Headaches

HEADACHES ARE THE MOST COMMON TYPE OF PAINS AT ALL. THEY DO NOT AVOID ANY AGE CATEGORIES. HEADACHE IS ALWAYS SUBJECTIVE A SUBJECTIVE SENSATION WHOSE INTENSITY CAN BE PERCEIVED AS
AS A MERE DISCOMFORT OR AS A ...AS AN UNBEARABLE SUFFERING.

Headaches do not originate in the brain (which contains no sensory fibres), they are the result of irritation of special sensory sensory nerve fibres and endings. Stimulation of any of these leads to the transmission of pain signals to the brain. The pain comes from the envelope of the brain, the walls of blood vessels, the hair of the head, the muscles of the neck and head. Pain is caused by pressure or expansion of these structures. Very often accompanied by vegetative symptoms and psychological difficulties.

The pain is of a varied character - throbbing, stabbing, burning, cutting, area or localized, unilateral or spreading to the surroundings. It can may be related to a variety of influences. Stress, menstrual cycle, certain foods, seasons, change in weather, physical or mental stress, ocular stimuli, body position, etc. A headache can arise when any system, organ or area of the body is affected.

DISTRIBUTION OF HEADACHES
Diagnosis of headaches is very difficult, it belongs in the hands of a neurologist!

Primary headaches

They have no organic basis that can be found by imaging (CT, MRI). They are classified according to manifestations, i.e. the type of pain:

  • Migraine - the best known type of headache, often preceded by aura.

According to the principles of TCM, migraines are classified based on the location of the pain into 3 main types:
gastric (pain localized in the frontal region).
- often associated with food allergies, accompanied by nausea, vomiting
- psychological manifestations: often fatigue, anxiety
gallbladder (throbbing pain, localised in the temples)
- induced by overeating, alcohol
- typical bitterness in the mouth, bloating, light-headedness
- psychological manifestations: depression, drowsiness, sometimes anger
renal (pain spreads from the back of the head to the root of the nose)
- associated with weather changes
- accompanied by pressure in the eyes, cold extremities, urge to urinate
- psychological manifestations: strong alteration, fear

  • tension headaches - linked to stressful situations, persistent psychological tension
  • cluster headache - unilateral stabbing short-termseizure-like pain of unclear cause
  • other primary headaches, transient and mixed types

Secondary headaches

They are linked to a specific cause:

  • head or neck injury
  • vascular disease of the intracranial or cervical spine
  • disease of the intracranial structures
  • infection, inflammation of the nervous system
  • pain caused by chemical  substances (medicines, drugs, alcohol, smoking) or their withdrawal
  • diseases of the facial or head structures (eyes, throat, neck, eyes, nose, sinuses, teeth, mouth)
  • psychiatric diseases
  • cranial neuralgia (pain emanating from the cranial nerves) and central facial pain
  • other diseases

The most important thing is to identify the triggering cause for secondary headaches and, if possible, eliminate it.

Important anamnestic information to determine the type of headache (what we ask)

  • circumstances of occurrence
  • frequency and intensity of pain
  • localisation, spread of pain
  • nature of pain
  • association with the menstrual cycle in women
  • relation to climatic influences, environmental changes
  • link to food, allergies
  • presence of aura
  • accompanying symptoms
  • what helps to soothe the pain
  • the effect of drugs and what kind of drugs

Warning (when to seek medical attention)

  • severe sudden headaches during physical exertion
  • new-onset pain in a client with a history of cancer
  • gradually increasing pain associated with vomiting, sleepiness, psychological changes
  • headache accompanied by fever and meningismus (irritation of the meninges)
  • the presence of any neurological symptomatology (impaired mobility, sensitivity)

 Prevention of headaches

  • plenty of fluids
  • plenty of healthy, undisturbed sleep
  • sport, exercise regime
  • limitation of TV, PC, long-term use mobile phone
  • avoiding risky foods
  • consider the use of hormonal contraceptives for girls and women
  • stress prevention
  • vision correction (eye examination)
  • do not underestimate head injuries (professional examination)
  • prevention of internal diseases (hypertension, obesity, digestive disorders)

Methods of treatment

  • classical allopathic medicines
  • psychotherapy, motivation
  • dietary measures - exclusion of allergens from food
  • physical therapy - rehabilitation, spinal mobilisation
  • acupuncture, acupressure
  • homeopathy
  • surgical solutions
  • phytotherapy

Classical therapy is guided by the type of headache. From the use of conventional analgesics to a one-time intervention of the type of salicylate, ibuprofen,
paracetamol, indomethacin, to prophylactic treatment, where the patient takes medication to prevent headache attacks. Here it isdrugs like antidepressants,  antiepileptics. The most common drugs used nowadays to treat acute migraine attacks are triptans.
